Authority to act form for a penalty notice or enforcement order

To help us answer your query, please quote the number on your penalty notice or enforcement order to the client contact officer handling your query.

If you do not know the penalty notice or the enforcement order number, we will ask for your name, vehicle registration or your licence number.

We can only discuss or correspond with the person named on the fine unless we have their authority to do otherwise. If you would like another person to act on your behalf, you can send us an authority to act form or phone us to verbally authorise another person.
