How do I complete the statutory declaration?

A statutory declaration form is provided with penalty reminder notices and camera detected penalty notices, for offences which can be transferred to another person.

Complete the statutory declaration telling us the name, address, date of birth and licence number of the person responsible.

The statutory declaration must be signed by the person named on the notice and witnessed by a Justice of the Peace or other authorised person.

Should I pay before the fine is transferred to the person responsible?

Do not send your payment before or with the statutory declaration. Once we transfer the fine to the person you tell us was responsible, we will send them a new penalty notice.

They then have a further 21 days in which to pay or finalise the fine. This will ensure demerit points are correctly applied.
